Sub-Acute Pediatric & Adult/Geriatric Care
Covenant Home Health of California specializes in caring for pediatric, adult and geriatric patients who have sub-acute health conditions. These are patients who need complex care or rehabilitative monitoring that is especially important for patients with sub-acute health issues.
Sub-acute care is usually recommended by the physician for patients with health issues that have been persistent for 1 to 3 months.
LVN/Respite Care
Covenant Home Health of California will provide Regional Center Consumers with Respite Care by Licensed Vocational Nurses. LVNs will be tasked to provide patients with health oversight while their loved ones or family members take a break from caregiving tasks.

- Weekend Respite
- Day-Time Care

Skilled Nursing
Well-trained, professional and compassionate – these are our skilled nurses at Covenant Home Health of California. With the direction of your physician, Skilled Nurses can provide you with the following health services in the home environment:
- RN Assessment & Case Management
- Wound Assessment and Care ( including Wound Vac)
- IV Therapy (Antibiotic, TPN, Lipids, Fluid Replacement)
- PICC Line Access, Care and Removal
- Central Line Care (BardPort, Triple lumen, Subclavian)
- Foley Catheter Insertion and Care
- Enteral and Parenteral Feedings
- Tracheostomy and Gastrostomy Tube
- Ventilator Management
- Pain Management
- Stroke Management
- Medication Management
- Anti-Coagulation Therapy and Monitoring
- Scheduling of Blood Draw and Management of Results
- Education on Disease Process, Treatment Regimen, Medication Administration
- Ostomy/Enterostomal Care (Appliance Information & Education, Support Group Education Patient & Caregiver Education)
- Experience Applying gor Medi-Cal EPSDT Program TAR Approval

Respiratory Therapy
Covenant Home Health of California’s Respiratory Therapists are licensed and experienced in caring for patients who have Chronic Respiratory Diseases. Some of the patients we care for are suffering from:

- Pneumothorax

Home Health Aide
Your Home Health Aide from Covenant Home Health of California will be assigned with your needs for total Personal Care. They can also handle light housekeeping tasks and meal preparation along with:
- Reminding the Client to take Medications
- Providing Companionship
- Bathing, Hair Washing and Shaving Assistance
- Mobility Assistance
- Running Light Errands
- Help the Client with Daily Personal Tasks (Bathing and Dressing)
- Assistance with Clothing Selection and Dressing
- Help the Client Maintain a Home Exercise Plan
- Assist with toileting (use of bedpan, commode or toilet)
- General Health Supervision
Physical Therapy
Physical Therapists will first conduct an assessment of your Rehabilitation Needs and will then recommend a therapy plan that may include one or a combination of the following modules:
- Therapeutic Exercises
- Transfer and Gait Training
- Relearning Self-Care Skills
- Post-Surgical Treatment and Care

Occupational Therapy

Speech Therapy

Our medical social workers will provide an assessment of Social & Emotional factors affecting your health. They can also provide Counseling and Community Resources Planning along with:

- Assistance with Inpatient and Outpatient Services
- Facilitating Advance Directives (Living Will or Power of Attorney)
- Case Management
- Nursing Home Placement Assistance (when it is no longer avoidable)
